  1. Print application
  2. Read conditions of use
  3. Complete and enclose a 2" x 2 1/2" photograph
  4. Enclose photocopy of acceptable proof of age (driver's license, Medicare card or birth certificate - one must have photo) or qualifying disability
  5. Mail to:

  • Bring two valid IDs (driver's license, Medicare card, or birth certificate), one of which must have your photograph.
  • Free photography services available
  • Notary not necessary
